Job summary

All Care and Comcare Home Health is seeking a compassionate Field Physical Therapist to deliver skilled PT services in patients' homes across our service area in California. You will perform SOC/ROC/Recert/DC evaluations, develop goals with the interdisciplinary team, educate patients and families, and travel to patient homes with a flexible schedule and supportive team.

Qualifications

• Active license/registration with the Physical Therapy Board of California.
• Graduate of an accredited Physical Therapy program.
• Strong documentation and organizational skills.
• Excellent communication and interpersonal abilities.
• Willingness to travel to patient homes for evaluations and treatment.
• Flexibility regarding work locations throughout our service area.

Responsibilities

• Provide comprehensive, skilled physical therapy services and interventions in accordance with physician orders.
• Perform SOC (start of care), ROC (resumption of care), Recert (recertifications), and DC (discharges).
• Assess patient needs, plan interventions, evaluate outcomes, and modify care to meet goals.
• Collaborate with the interdisciplinary team to develop individualized patient goals.
• Educate and train patients, families, and caregivers on programs and home exercises.

Position: Field Physical Therapist
