Job summary

All Care and Comcare Home Health is seeking a compassionate Physical Therapist to provide skilled services in patients' homes with a flexible schedule and supportive team environment.

The Field Physical Therapist will perform SOC/ROC/Recert/DC evaluations, develop individualized care plans, educate patients and families, and collaborate with an interdisciplinary team to meet therapy goals across our service area in California.

Qualifications

• Licensed as a Physical Therapist with California license/registration.
• Ability to document patient progress clearly and accurately.
• Strong communication with patients and care team.
• Willingness to travel to patient homes within service area.

Responsibilities

• Provide comprehensive physical therapy services per physician orders.
• Perform SOC, ROC, Recert, and DC evaluations and documentation.
• Assess patient needs, plan and adjust interventions to meet goals.
• Collaborate with the interdisciplinary team on patient goals.
• Educate patients and caregivers on home exercise programs.

Position: Field Physical Therapist

Key Responsibilities:

• Provide comprehensive, skilled physical therapy services and interventions in accordance with physician orders.
• Perform start of care (SOC) evaluations, resumption of care (ROC), recertifications (Recert), and discharges (DC).
• Assess patient skilled needs, plan interventions, evaluate outcomes, and modify care to meet physical therapy goals.
• Collaborate with the interdisciplinary team to develop and evaluate individualized patient goals.
• Educate, instruct, and train patients, families, and caregivers on physical therapy programs and home exercise regimens.

Qualifications:

• Active license/registration with the Physical Therapy Board of California.
• Graduate of an accredited Physical Therapy program.
• Strong documentation and organizational skills.
• Excellent communication and interpersonal abilities.
• Willingness to travel to patient homes for evaluations and treatment.
• Flexibility regarding work locations throughout our service area.
