Job summary

All Care and Comcare Home Health is seeking a compassionate Field Physical Therapist to join our growing team. This role provides high-quality, individualized care in patients' homes with a flexible schedule and supportive team environment.

The candidate will perform SOC evaluations, ROC, Recertifications, and discharges, collaborate with the interdisciplinary team, and educate patients and families on home exercise programs.

Qualifications

• Active license/registration with the Physical Therapy Board of California.
• Strong documentation and organizational skills.
• Excellent communication and interpersonal abilities.
• Willingness to travel to patient homes for evaluations and treatment.
• Flexibility regarding work locations throughout our service area.

Responsibilities

• Perform start of care (SOC) evaluations, resumption of care (ROC), recertifications (Recert), and discharges (DC).
• Assess patient skilled needs, plan interventions, evaluate outcomes, and modify care to meet physical therapy goals.
• Collaborate with the interdisciplinary team to develop and evaluate individualized patient goals.
• Educate, instruct, and train patients, families, and caregivers on physical therapy programs and home exercise regimens.

Qualifications:

• Active license/registration with the Physical Therapy Board of California.
• Graduate of an accredited Physical Therapy program.
