Job summary

All Care and Comcare Home Health in California seeks a compassionate Field Physical Therapist to deliver skilled services at patients’ homes. You will assess, treat, and monitor progress with physician orders, SOC/ROC/Recert/DC processes, and collaborate with the care team.

Travel to patient homes is required across our service area. A California PT license, current CPR, and reliable transportation are essential.

Qualifications

• Active license/registration with the Physical Therapy Board of California.
• Graduate of an accredited Physical Therapy program.
• Strong documentation and organizational skills.
• Excellent communication and interpersonal abilities.
• Willingness to travel to patient homes for evaluations and treatment.
• Flexibility regarding work locations throughout our service area.

Responsibilities

• Provide skilled physical therapy services per physician orders.

Position: Field Physical Therapist

Key Responsibilities:

• Provide comprehensive, skilled physical therapy services and interventions in accordance with physician orders.
• Perform start of care (SOC) evaluations, resumption of care (ROC), recertifications (Recert), and discharges (DC).
• Assess patient skilled needs, plan interventions, evaluate outcomes, and modify care to meet physical therapy goals.
• Collaborate with the interdisciplinary team to develop and evaluate individualized patient goals.
• Educate, instruct, and train patients, families, and caregivers on physical therapy programs and home exercise regimens.
