Job summary

Laurel Park Rehabilitation And Healthcare Center in Elizabeth City, NC, is hiring a PT. The role offers full-time or part-time opportunities to provide comprehensive physical therapy, evaluations, and customized treatment plans within an interdisciplinary team.

The successful candidate will hold a PT degree recognized by the APTA, maintain current licensure, and participate in ongoing documentation and patient care activities in a supportive environment.

Qualifications

Graduate of an accredited university with a B.S., Doctorate, MA or MS in Physical Therapy recognized by the APTA.
Holds current license/registration in the state (as applicable).

Responsibilities

Provide a comprehensive physical therapy evaluation based on MD’s orders.
Document findings on the evaluation form per guidelines.
Develop a comprehensive treatment plan with goals, frequency, duration and modalities.
Obtain MD orders for evaluations, treatments, re-certifications and discharges.
Deliver treatments using appropriate modalities and adjust as needed.
Document daily participation and progress in weekly notes.
Update treatment plans and discharge summaries timely.
