Qualifications

• Graduate of an accredited university with a B.S., Doctorate, MA or MS in Physical Therapy.
• Holds current license and/or registration in the state as applicable.

Responsibilities

• Provide comprehensive PT evaluation based on MD orders.
• Document findings on the evaluation form and adhere to guidelines.
• Develop and implement a comprehensive treatment plan with goals, frequency and modalities.
• Ensure MD orders for evaluations, treatments, re-certifications and discharges.
• Deliver treatments using modalities and modify plans as needed while following precautions.
• Maintain daily progress notes and update discharge summaries when indicated.
• Coordinate care with the interdisciplinary team and participate in facility meetings.
• Complete documentation and billing ethically for PT services.
