Qualifications

• Graduate of an accredited university with a PT degree (B.S., DPT, MA or MS) recognized by the APTA.
• Current license/registration in North Carolina.

Responsibilities

• Provide comprehensive physical therapy evaluations based on physician orders.
• Develop and document treatment plans with long- and short-term goals, frequency, duration, and modalities.
• Obtain MD orders for evaluations, treatments, re-certifications and discharges.
• Deliver treatments and adjust plans as needed while adhering to safety precautions.
• Document patient participation and progress weekly, and update care plans and discharge summaries.
• Attend facility and rehab meetings and collaborate with the interdisciplinary team.
• Supervise junior staff or students as needed and co-sign notes.

Education

B.S./DPT/MA/MS in Physical Therapy (ACCEPTED).
