Job summary

Laurel Park Rehabilitation And Healthcare Center in Elizabeth City, NC is seeking a Physical Therapist for full-time or part-time roles. You will perform comprehensive PT evaluations, develop and implement treatment plans, and ensure proper documentation and MD-order compliance.

The position offers a collaborative rehab team and opportunities for growth. Requirements include a PT degree and active state license.

Qualifications

• Graduate of an accredited university with a B.S., Doctorate, MA or MS in Physical Therapy recognized by the APTA.
• Holds current license and/or registration in the state (as applicable).

Responsibilities

• Provide comprehensive physical therapy evaluations based on MD orders.
• Develop comprehensive treatment plans including long- and short-term goals, frequency, duration, and modalities.
• Ensure MD orders are obtained for evaluations, treatments, re-certifications and discharges.
• Deliver treatment with appropriate modalities and modify as indicated while adhering to precautions.
• Document daily participation and progress on weekly progress notes and timely discharge summaries.
• Complete care plans and information needed for MDS as required by the facility.
• Attend rehab meetings and participate in facility committees as indicated.
• Supervise junior staff or students and co-sign notes as required.
