Qualifications

• Graduate of an accredited university with a B.S., Doctorate, MA or MS in Physical Therapy recognized by the APTA.
• Holds current license and/or registration in the state (as applicable).

Responsibilities

• Provide a comprehensive physical therapy evaluation based on MD’s orders.
• Develop and document a comprehensive treatment plan with goals, frequency, duration and modalities.
• Ensure MD orders are obtained for evaluations, treatments, re-certifications and discharges.
• Deliver treatment using modalities and adjust as needed, while observing precautions.
• Document daily participation, progress and update treatment plans and discharge summaries.
• Attend facility meetings and participate in rehab program activities.
• Supervise junior staff or students as needed and co-sign notes.
