Job summary

Qualifications

• Bachelor's degree in Physical Therapy required; Master’s or Doctorate preferred.
• Licensed as a Physical Therapist in the state of Ohio; continuing education as required.

Responsibilities

• Evaluate residents to determine PT needs and rehab potential.
• Develop and update individualized treatment plans to improve mobility and independence.
• Supervise PT assistants and provide staff training.
• Communicate progress with physicians, nursing staff, residents, and families.
• Complete accurate documentation in line with policies and payer requirements.
