Job summary

Bridge Crest Post Acute is seeking a dedicated Physical Therapist Assistant (PTA) to join our care team. This position involves collaborating with the Physical Therapist to provide high-quality resident care in a supportive, team-driven environment.

The PTA will handle documentation, patient education, and adherence to infection control protocols while supporting discharge planning and staff training as needed.

Qualifications

• Associate Degree from an accredited PTA program
• Proficient in Net Health and Point Click Care
• Ability to read technical procedures and policy manuals
• Ability to communicate effectively with residents, families, and colleagues

Responsibilities

• Treat residents as directed by Physical Therapist
• Complete timely, accurate documentation per facility and payer requirements
• Maintain a safe treatment environment and infection control protocols
• Participate in care conferences and rehabilitation meetings
• Educate residents and families on treatment goals and home exercise programs
• Coordinate with supervising PT for progress updates and discharge planning
