Job summary

Continuum Therapy Partners seeks a Physical Therapist for a full-time on-site role at Hartwell Health and Rehab in Garner, NC. The therapist will evaluate and implement individualized treatment plans, coordinate with other disciplines, and monitor patient progress to adjust care plans accordingly.

The role includes educating patients and families on home exercise programs, injury prevention, and ongoing care.

Qualifications

• DPT degree required.
• Current, unencumbered state Physical Therapy License; must pass NPTE.
• 1 year of previous experience as a Physical Therapist preferred.

Physical Therapist Requirements & Responsibilities:

• Doctor of Physical Therapy (DPT) degree required
• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)
