Continuum Therapy Partners in Huber Heights, OH is seeking a licensed Physical Therapist (DPT) to join a daytime, full-time role at Wayne Health & Rehab Center. You will evaluate patients, develop and implement individualized treatment plans, and coordinate care with other disciplines.

You will document progress, educate patients and families on home programs, and contribute to a supportive, patient-centered environment. We offer a comprehensive benefits package, licensure reimbursement, and PTO.

Formación

• DPT degree required.
• Current, unencumbered state Physical Therapy License; must pass the NPTE.
• 1 year of previous experience as a Physical Therapist preferred.

Physical Therapist Requirements & Responsibilities:

• Doctor of Physical Therapy (DPT) degree required
• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)
• 1 year of previous experience as a Physical Therapist preferred
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines
• Documents patient progress and adjusts plans of care based on clinical outcomes
