Post Rehabilitation Services is seeking a dedicated Physical Therapist Assistant (PTA) to join the Civita Care Center at Long Ridge in Stamford, CT. The PTA will work with licensed Physical Therapists to deliver hands-on care aimed at improving mobility, strength, and functional goals for residents.

• Active Connecticut PTA license required.
• New graduate PTAs encouraged to apply.
• Experience in skilled nursing or post-acute settings preferred but not required.

Responsibilities

• Deliver physical therapy treatments under the direction of a licensed Physical Therapist.
• Guide residents through strengthening, balance, gait, transfer, and mobility activities.
• Follow individualized treatment plans established by the supervising PT.
• Monitor resident response to treatment and communicate progress or changes.
• Reinforce therapeutic exercises and safe movement techniques.
• Complete required clinical documentation accurately and on time.
• Collaborate with therapy, nursing, and other healthcare professionals to support resident care.

Post Rehab is hiring a Physical Therapist Assistant (PTA) to become part of the therapy team at Civita Care Center at Long Ridge. Work alongside licensed Physical Therapists to provide hands‑on care that helps residents improve mobility, increase strength, and make meaningful progress toward their functional goals.

What You'll Do

• Deliver physical therapy treatments under the direction of a licensed Physical Therapist
• Guide residents through strengthening, balance, gait, transfer, and mobility activities
