Job summary

Post Rehabilitation Services is seeking a licensed Physical Therapist (PT) to join Civita Care Center Cheshire in Cheshire, CT. This full-time role focuses on evaluating residents, designing individualized plans, and guiding mobility and independence in a skilled nursing and post-acute setting.

You will work with a collaborative rehabilitation team, provide patient-centered therapy, and document progress while supporting ongoing professional growth and quality care for residents.

Qualifications

• Active Connecticut PT license required.
• Graduate of an accredited Physical Therapy program.
• Strong clinical judgment and communication skills.
• Experience in skilled nursing or rehabilitation is preferred.

Responsibilities

• Perform PT evaluations and develop plans of care.
• Provide skilled treatment focused on strength, balance, gait, transfers, endurance, and mobility.
• Establish functional goals and monitor resident progress.
• Educate residents and caregivers on exercises, mobility strategies, and safety techniques.
• Complete accurate clinical documentation.
• Collaborate with PTAs, OT, SLP, nursing, and other team members.

Post Rehab is seeking a Physical Therapist (PT) to join the rehabilitation team at Civita Care Center Cheshire. This full-time opportunity offers the chance to work directly with residents while helping them improve mobility, rebuild strength, and achieve greater functional independence in a skilled nursing and post-acute care environment.

What You'll Do

• Perform physical therapy evaluations and develop individualized plans of care
• Provide skilled treatment focused on strength, balance, gait, transfers, endurance, and mobility
• Establish functional goals and monitor resident progress throughout treatment
• Modify treatment approaches based on clinical findings and resident needs
• Educate residents and caregivers on exercises, mobility strategies, and safety techniques
• Complete accurate and timely clinical documentation
• Collaborate with PTAs, OT, SLP, nursing, and other members of the interdisciplinary team
