Post Rehabilitation Services is hiring a full‑time Physical Therapist at Civita Care Center at Meadowbrook. Help skilled nursing residents regain mobility through care that considers both their recovery needs and the activities they hope to return to. You'll guide treatment from evaluation through discharge, working closely with residents and the broader care team.

What You’ll Do

• Evaluate physical function, including strength, balance, walking, and transfer abilities.
• Establish individualized treatment plans with measurable goals informed by medical history, precautions, and prior function.
• Deliver therapeutic exercise, gait training, and functional activities that support everyday movement.
• Recommend appropriate mobility equipment and educate residents and caregivers on safe techniques.
• Assess progress, revise interventions, and provide clinical supervision to PTAs.
• Maintain thorough documentation and coordinate with nursing and rehabilitation colleagues on care needs and discharge planning.
