Post Rehabilitation Services is seeking a licensed Physical Therapist to join Civita Care Center at Cheshire, CT. This full-time role offers an hourly wage of $45–$50, comprehensive benefits, and the opportunity to help residents regain mobility and independence through individualized therapy plans.

You will evaluate movement, set functional goals, improve strength and balance, ensure safe transfers, reassess progress, and collaborate with nursing and rehab staff. New grads are welcome to apply.

Qualifications

• Active Connecticut Physical Therapist license.

Post Rehabilitation Services is hiring a Physical Therapist at Civita Care Center at Cheshire. You'll help residents regain mobility after illness or injury and address ongoing physical challenges through care tailored to their daily needs.

What You’ll Do

• Evaluate residents' movement and establish functional treatment goals
• Provide therapy to improve strength, endurance, balance, and walking
• Address transfer safety and recommend appropriate mobility devices
• Reassess residents and update care plans as their abilities change
• Document outcomes and coordinate recommendations with nursing and the rehab team
