Job summary

Mill Creek Center for Nursing & Rehabilitation is seeking a compassionate Physical Therapist Assistant (PTA) to join our rehabilitation team in a part-time Day Shift role, providing hands-on therapy to residents.

You will follow individualized treatment plans, supervise under a licensed PT, document progress, and collaborate with nursing staff, families, and other therapists to promote safety, mobility, and independence.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Current North Carolina PTA license.
• Ability to work effectively as part of a multidisciplinary team.

Responsibilities

• Provide hands‑on therapy under the supervision of a licensed Physical Therapist.
• Carry out individualized treatment plans designed to meet each resident’s goals.
• Provide individual and group therapy as appropriate.
• Monitor resident progress and communicate changes with the therapy and nursing teams.
• Complete accurate and timely clinical documentation.
• Assist with discharge planning and resident/family education.
• Collaborate with residents, families, nursing staff, therapists, and other care team members.
• Participate in care conferences and facility initiatives.
• Promote resident safety, dignity, mobility, and independence.

Mill Creek Center for Nursing & Rehabilitation is seeking a compassionate and skilled Physical Therapist Assistant (PTA) to join our supportive and collaborative rehabilitation team. In this part-time role, you'll help residents improve strength, mobility, independence, and quality of life through personalized, hands-on care.

What You'll Do

• Provide hands‑on therapy under the supervision of a licensed Physical Therapist

• Carry out individualized treatment plans designed to meet each resident’s goals

What We’re Looking For

• Graduate of an accredited Physical Therapist Assistant program

• Current North Carolina PTA license

• Previous skilled nursing or long‑term care experience preferred, but not required

• Strong communication, organization, and time‑management skills

• Ability to work effectively as part of a multidisciplinary team

• Compassionate, dependable, and resident‑focused approach

• Ability to safely assist residents with mobility and therapeutic activities
