Job summary

Mill Creek Center for Nursing & Rehabilitation is seeking a licensed Physical Therapist Assistant (PTA) to join our rehabilitation team in a skilled nursing setting. You’ll work under the supervision of a Physical Therapist to help residents improve strength, mobility, independence, and quality of life.

This full-time, day-shift position emphasizes collaboration with the interdisciplinary team, documentation, discharge planning, and patient education to support residents’ goals and well-being.

Qualifications

• Active PTA license is required.
• Experience in skilled nursing/long-term care is preferred.
• Ability to work under supervision of a Physical Therapist and with the care team.

Responsibilities

• Provide hands-on therapy under the direction of a Physical Therapist.
• Follow individualized treatment plans to support resident goals.
• Deliver individual and group therapy as appropriate.
• Monitor progress and communicate changes to the PT and team.
• Complete daily treatment notes and required documentation.
• Assist with discharge planning and transition of care.
• Collaborate with nursing, therapy staff, physicians, residents, and families.
• Participate in care conferences and facility initiatives.
• Promote resident safety, dignity, and independence.
