Mill Creek Center for Nursing & Rehabilitation invites qualified Physical Therapy Assistants (PTAs) to join our PRN team. You will work under a Physical Therapist to provide both individual and group therapy to residents, while documenting progress and assisting with discharge planning.

Qualifications

• Graduate of an accredited PTA program.
• Current state licensure as a PTA.
• Prior LTC/SNF experience is a plus, but not required.
• Ability to lift up to 50 lbs and assist residents as needed.
• Strong teamwork and time management skills.

Responsibilities

• Follow treatment plans as directed by the Physical Therapist.
• Provide one-on-one and group therapy to residents.
• Track daily notes, treatment progress, and billing.
• Assist with discharge planning and patient education.
• Communicate with therapy team, nursing, and families.
• Participate in care meetings and facility initiatives.
