Physical Therapist Assistant (PTA)
Full-Time | Part-Time | PRN
We are seeking a motivated, compassionate, and patient-focused Physical Therapist Assistant (PTA) to join our rehabilitation team. Under the direction of a licensed Physical Therapist, you will provide skilled therapeutic services designed to help residents achieve their functional goals, improve mobility, and maintain independence.

Key Responsibilities
- Provide skilled therapeutic treatments under the direction of a licensed Physical Therapist.
- Carry out individualized treatment plans in accordance with physician orders and established care plans.
- Monitor resident response to treatment and communicate progress, concerns, and changes to the supervising therapist.
- Accurately and timely document treatments, resident progress, and daily charges.
- Maintain established productivity, documentation, and quality standards.
- Participate in interdisciplinary team meetings, care conferences, utilization reviews, and care planning activities as assigned.
- Educate residents, families, and nursing staff regarding restorative programs, home exercise programs, and therapeutic activities as directed.
- Maintain clean, safe, and organized therapy areas and equipment.
- Report equipment issues, safety concerns, or maintenance needs to the appropriate personnel.
- Participate in facility performance improvement and quality initiatives.
- Travel or float between facilities as assigned based on resident and operational needs.
- Maintain compliance with Medicare, reimbursement, documentation, and applicable regulatory requirements.
- Promote resident safety, dignity, independence, and quality of life.
Qualifications
- Graduate of an accredited Physical Therapist Assistant program.
- Current state license, certification, or registration as a PTA, as applicable.
- Knowledge of Medicare, therapy documentation, and reimbursement requirements preferred.
- Strong communication, organization, and documentation skills.
- Ability to work effectively within an interdisciplinary healthcare team.

Physical Requirements
- Ability to stand, walk, bend, lift, and assist residents throughout the workday.
- Ability to safely lift, carry, and support up to 50 pounds, as required.
- Ability to assist residents with transfers, mobility activities, and therapeutic exercises.
- Ability to manage the physical and emotional demands of patient care.
- Comfortable working in a clinical and skilled nursing environment.
