Job summary

Mill Creek Center for Nursing & Rehabilitation is seeking a compassionate Physical Therapist Assistant (PTA) to join our rehabilitation team in a part-time, day-shift role. The PTA will help residents improve strength, mobility, independence, and quality of life through hands-on, individualized care in a skilled nursing/long-term care setting.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Current North Carolina PTA license.
• Experience in skilled nursing/long-term care is preferred but not required.
• Strong communication, organization, and time-management skills.
• Ability to work effectively as part of a multidisciplinary team.

Responsibilities

• Provide hands-on therapy under the supervision of a licensed Physical Therapist.
• Carry out individualized treatment plans designed to meet each resident’s goals.
• Provide individual and group therapy as appropriate.
• Monitor resident progress and communicate changes with the therapy and nursing teams.
• Complete accurate and timely clinical documentation.
• Assist with discharge planning and resident/family education.
• Collaborate with residents, families, nursing staff, therapists, and other care team members.
• Participate in care conferences and facility initiatives.
• Promote resident safety, dignity, mobility, and independence.
