Setting: Skilled Nursing / Long-Term Care
Provide hands‑on therapy services under the direction and supervision of a Physical Therapist
Follow individualized treatment plans designed to support each resident’s goals
Provide individual and group therapy as appropriate
Monitor resident progress and communicate changes to the Physical Therapist and interdisciplinary team
Complete accurate and timely daily treatment notes and required documentation
Assist with discharge planning, resident education, and transition of care
Collaborate with nursing, therapy staff, physicians, residents, and families
Participate in care conferences and other facility initiatives
Promote resident safety, dignity, independence
