Constant Care Health Services in The Bronx is seeking highly trained Physical Therapist Assistants (PTA) to join our 5 Star Skilled Nursing Facilities. You will assist PTs in providing patient-centered care and help residents achieve their treatment goals.

Responsibilities

• Assist Physical Therapists (PT) in delivering patient care.

• Deliver personalized care to elderly residents.
• Follow care plans established after initial evaluation to set goals, frequency, and modalities.
• Perform other duties as assigned.
