Job summary

Nexus Health is seeking a dedicated Physical Therapist Assistant to work under the supervision of a licensed Physical Therapist in a skilled nursing or long-term care setting. You will implement treatment plans, assist residents with mobility, strength, balance, and independence, and document progress per physician orders.

Qualifications include an active Illinois PTA license and knowledge of current treatment practices. SNF/long-term care experience is preferred, and PCC familiarity is a plus.
