Job summary

Nexus at Godfrey, in Illinois, seeks a dedicated Physical Therapist to develop individualized treatment plans and help residents regain independence while maintaining quality of life. You will assess patient evaluations, design rehab programs, and administer PT treatments in a supportive, team-oriented facility.

Ideal candidates hold an Illinois PT license, with SNF/long-term care experience preferred, and familiarity with PCC documentation.

Qualifications

• Active/Valid Illinois license in Physical Therapy
• SNF/Long term care experience preferred
• Current knowledge of treatment practices
• Knowledge of PCC Electronic Documentation System preferred
