Job summary

Nexus at Alton is seeking a dedicated Physical Therapist to develop individualized treatment plans, assess patient evaluations, and help residents regain independence through targeted rehabilitation.

You will create personalized rehabilitation programs, administer prescribed PT treatments, and monitor progress to improve function and quality of life in a SNF/long-term care setting.

Qualifications

• Active/Valid Illinois license in Physical Therapy.
• SNF/Long term care experience preferred.
• Current knowledge of treatment practices.
• Knowledge of PCC Electronic Documentation System preferred.

Responsibilities
