Nexus at Wood River in Wood River, IL is seeking a dedicated Physical Therapist to develop individualized treatment plans that help residents regain independence and participate safely in daily activities, while enhancing quality of life.
You will assess evaluations, create rehabilitation programs, and administer evidence-based therapies, collaborating with a supportive team in a SNF setting; a valid Illinois PT license and PCC EHR knowledge are preferred.
Position Overview:
Develop individualized treatment plans to help residents regain independence, prevent decline, and safely participate in daily activities while enhancing quality of life.
Key Responsibilities:
Assessing and interpreting patient evaluations and test results.
Developing treatment plans using a variety of treatment techniques.
Creating personalized rehabilitation programs for patients.
Administering medically prescribed physical therapy treatments to relieve pain, improve or restore function, and promote healing.

Preferred Qualifications:
Active/Valid Illinois license in Physical Therapy
SNF/Long term care experience preferred.
Current knowledge of treatment practices
Knowledge of PCC Electronic Documentation System preferred.
