Job summary

Nexus Health in Illinois is seeking a Physical Therapist to help residents regain independence by developing individualized treatment plans and providing skilled PT services across the facility.

You will assess evaluations, select techniques, and create tailored rehabilitation programs to relieve pain, restore function, and support healing, with a collaborative team and strong emphasis on patient quality of life.

Qualifications

• Active/Valid Illinois license in Physical Therapy
• SNF/Long term care experience preferred.
• Current knowledge of treatment practices
• Knowledge of PCC Electronic Documentation System preferred.
