Continuum Therapy Partners is seeking a Physical Therapist Assistant to join our team at Heritage Manor in Chisholm, MN. The role is full-time and daytime, offering a patient-focused, therapist-led environment with an emphasis on professional growth.

You will implement treatment plans under a PT, document progress, and educate patients on home exercise programs and assistive devices. State licensure and a valid PTA credential are required or preferred depending on experience.

Qualifications

• Associate degree in Physical Therapist Assistant program is required.
• Current, unencumbered state PTA license is required.
• 1 year of PTA experience is preferred.

Responsibilities

• Implements individualized treatment plans under the supervision of a Physical Therapist.
• Documents patient progress and reports changes in patient status to the supervising PT.
• Educates patients on home exercise programs and proper use of assistive devices.

Physical Therapist Assistant Requirements & Responsibilities

• Associate Degree in Physical Therapist Assistant Program required
