Job summary

Traditional Home Health & Hospice in Northeast Pennsylvania seeks a qualified Physical Therapist Assistant to provide home-based therapy in Old Forge and surrounding areas. You will implement PT plans, educate patients on exercises, and coordinate with the clinical team to ensure progress under a standard care plan.

The role is full-time, Monday to Friday, 8:30am–5:00pm, with driving to patient homes and a supportive team environment.

Qualifications

• Must possess a current, valid Pennsylvania PTA license and CPR certification.
• Valid driver's license and ability to travel to case assignments.
• Comfortable working in home setting and independently with minimal supervision.
• Strong interpersonal and communication skills.

Responsibilities

• Provide therapeutic treatment modalities and home PT including ROM, strengthening, balance, gait and transfer training.
• Instruct patients on home exercise, home safety, incision care, and medications to reach goals.
• Maintain communication with ordering providers and clinical team to promote progress.
• Participate in quality improvement activities as assigned.
• Timely and accurate documentation of patient assessments and interactions.

Physical Therapist Assistant

Job Location: Old Forge, PA and surrounding areas

Our Physical Therapist Assistants see patients in the most ideal setting - the patient's home!

Our office is located in Dunmore, PA. At Traditional, we do our best to keep each PTA's case load within a sensible geographic territory.

Shift(s): Full-time positions available. Monday through Friday 8:30am to 5:00pm

Duties and Responsibilities include, but not limited to the following:

• In accordance with PT established Care Plan, provide therapeutic treatment modalities and implement home Physical Therapy including strengthening and ROM exercise, balance, gait, transfer training.
• Provide instruction to patients on topics such as home exercise, home safety, incision care, and medications to achieve patient goals
• Maintain communication with ordering providers, clinical team, and especially to primary PT to promote progress toward goals or seek direction when needed
• Participate in quality improvement activities as assigned
• Timely and accurately document (EMR) all patient assessments, interactions, etc

Requirements

• Must possess a current, valid Pennsylvania PTA license and have current CPR certification
• Valid driver's license and ability to travel to case assignments
• Must be comfortable working in home setting
• Ability to work independently with minimal supervision and manage time effectively
• Strong interpersonal and communication skills
