Traditional Home Health & Hospice is seeking a Physical Therapist Assistant to provide in-home physical therapy in Archbald, PA and surrounding areas. The PTA will work full-time, typically Monday–Friday, 8:30am–5:00pm, with support from a Dunmore-based office to manage caseload in a sensible geographic territory.

The role requires PA PTA licensure and CPR certification, a valid driver’s license, and comfort working in patients’ homes.

Qualifikationen

• Valid driver's license and ability to travel to assignments.
• Comfort working in home setting; independent with minimal supervision.
• Strong interpersonal and communication skills.

• Provide home PT modalities per established care plan, including ROM, strengthening, balance, gait and transfers.
• Instruct patients on home exercise, home safety, incision care, and medications to reach goals.
• Maintain communication with providers and clinical team to track progress and seek direction as needed.
• Participate in quality improvement activities as assigned.
• Timely, accurate documentation of patient assessments and interactions in EMR.

Shift(s):

Full-time positions available. Monday through Friday 8:30am to 5:00pm
Our office is located in Dunmore, PA. At Traditional, we do our best to keep each PTA's case load within a sensible geographic territory.

Duties and Responsibilities include, but not limited to the following:

• In accordance with PT established Care Plan, provide therapeutic treatment modalities and implement home Physical Therapy including strengthening and ROM exercise, balance, gait, transfer training.
• Provide instruction to patients on topics such as home exercise, home safety, incision care, and medications to achieve patient goals
• Maintain communication with ordering providers, clinical team, and especially to primary PT to promote progress toward goals or seek direction when needed
• Participate in quality improvement activities as assigned
• Timely and accurately document (EMR) all patient assessments, interactions, etc

Requirements:

• Must possess a current, valid Pennsylvania PTA license and have current CPR certification
• Valid driver's license and ability to travel to case assignments
• Must be comfortable working in home setting
• Ability to work independently with minimal supervision and manage time effectively
• Strong interpersonal and communication skills
