Job summary

Traditional Home Health Care in Archbald, PA is seeking a Physical Therapist Assistant to provide in-home therapy from Monday to Friday, 8:30am to 5:00pm, serving Archbald and surrounding areas.

You will implement care plans, guide patients on home exercises, document in EMR, and collaborate with the clinical team to meet patient goals. Requires PA PTA license and CPR, driver’s license, and comfort with home-based care.

Qualifications

• Must possess a current, valid Pennsylvania PTA license and CPR certification.
• Valid driver's license and ability to travel to case assignments.
• Comfortable working in home setting.
• Ability to work independently with minimal supervision and manage time effectively.
• Strong interpersonal and communication skills.

Responsibilities

• Deliver home Physical Therapy per Care Plan including strengthening and ROM exercises, balance and gait training.
• Instruct patients on home exercise, home safety, incision care, and medications to achieve goals.
• Communicate with ordering providers and clinical team to progress toward goals.
• Participate in quality improvement activities as assigned.
• Document all patient assessments and interactions in EMR in a timely, accurate manner.

Job Title:

Job Location:

Shift(s):

Full-time positions available. Monday through Friday 8:30am to 5:00pm

Duties and Responsibilities include, but not limited to the following:

• In accordance with PT established Care Plan, provide therapeutic treatment modalities and implement home Physical Therapy including strengthening and ROM exercise, balance, gait, transfer training.
• Provide instruction to patients on topics such as home exercise, home safety, incision care, and medications to achieve patient goals
• Maintain communication with ordering providers, clinical team, and especially to primary PT to promote progress toward goals or seek direction when needed
• Participate in quality improvement activities as assigned
• Timely and accurately document (EMR) all patient assessments, interactions, etc

Requirements
