Job summary

Traditional Home Health & Hospice in Northeast Pennsylvania is seeking a Physical Therapist Assistant to provide home-based care in Old Forge, PA and surrounding areas. This is a full-time role with a 8:30am–5:00pm schedule, and the office is located in Dunmore, PA.

We manage caseloads across a sensible geographic territory. You will implement PT plans, educate patients on home exercises and safety, coordinate with clinicians, and document all patient interactions in an EMR.

Qualifications

• Must possess a current, valid Pennsylvania PTA license.
• CPR certification required.
• Valid driver's license and ability to travel to case assignments.
• Comfortable working in home setting.
• Ability to work independently with minimal supervision and manage time effectively.

Responsibilities

• Provide home physical therapy including strengthening, ROM, balance, gait, and transfer training.
• Instruct patients on home exercise, safety, incision care, and medications.
• Maintain communication with ordering providers and clinical team.
• Participate in quality improvement activities.
• Timely and accurate documentation in EMR.
