Allegheny Health Network Healthcare@Home in Erie, PA, seeks a Home Health PTA to deliver skilled therapy to patients in their homes. You will work under a licensed PT, helping patients regain mobility and independence with therapeutic exercises, gait training, transfers, and assistive devices.

The role requires a PTA license, at least 1 year of PTA experience, and a valid driver’s license for ~20% travel within the territory. A $5,000 first-year success bonus is available as part of the package.

Qualifications

• Active Physical Therapist Assistant (PTA) license in the state you work.
• One year of PTA experience preferred to deliver quality patient care.

Responsabilités

• Provide compassionate, patient-centered PTA services in the home health setting under the direction of a licensed Physical Therapist.
• Support patients in restoring mobility and independence through therapeutic exercises and gait training.
• Educate patients and caregivers on safe use of mobility devices and monitor progress for care team updates.
• Maintain thorough, compliant EMR documentation of patient status and care delivered.
• Travel to patient homes within the assigned territory and work independently.

What You’ll Do:

• Provide compassionate, patient-centered Physical Therapy services in the home health setting under the direction of a licensed Physical Therapist.
• Support patients in restoring mobility, reducing pain, and maximizing functional independence through therapeutic exercises, gait training, transfer training, muscle re-education, and prosthetic or orthotic support.
• Educate patients and caregivers on the safe use of mobility and assistive devices, while monitoring progress and communicating patient responses and condition changes to the care team.
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
• Work independently, but never alone
• Travel to patient homes within assigned territory

PTA Qualifications

• Active Physical Therapist Assistant, PTA license in the state you work
• One year of experience as a Physical Therapist Assistant and the clinical expertise to deliver quality patient care
• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
