Responsibilities

• Provide physical therapy services according to a physician’s plan of care
• Initiate therapy programs and instruct personnel and family/caregivers as appropriate
• Prepare and submit clinical and progress summaries
• Provide in-service education for nursing staff as needed
• Participate in discharge planning
• Supervise Physical Therapy Assistants per policy and regulations
• Ensure compliance with legal and regulatory requirements
• Additional duties as assigned

Qualifications

• Graduate of an approved physical therapist education program with current licensure
• Minimum 2 years of adult physical therapy experience
• Home health experience preferred
• Commitment to clinical and documentation excellence
