Job summary

All Care and Comcare Home Health is seeking a compassionate Field Physical Therapist to deliver skilled services in patients' homes. The role offers a flexible schedule, a supportive team, and opportunities for growth within a growing home health organization.

The Physical Therapist will perform SOC/ROC/Recert/DC, assess needs, develop goals, and educate patients and families on home exercise programs. A CA PT license and strong communication are required.

Qualifications

• Active California Physical Therapy license.
• Graduate of an accredited Physical Therapy program.
• Strong documentation and organizational skills.
• Excellent communication and interpersonal abilities.
• Willingness to travel to patient homes.
• Flexible with work locations within service area.

Responsibilities

Position:

Field Physical Therapist

Key Responsibilities:

• Provide comprehensive, skilled physical therapy services and interventions in accordance with physician orders.
• Perform start of care (SOC) evaluations, resumption of care (ROC), recertifications (Recert), and discharges (DC).
• Assess patient skilled needs, plan interventions, evaluate outcomes, and modify care to meet physical therapy goals.
• Collaborate with the interdisciplinary team to develop and evaluate individualized patient goals.
• Educate, instruct, and train patients, families, and caregivers on physical therapy programs and home exercise regimens.

Qualifications:

• Active license/registration with the Physical Therapy Board of California.
• Graduate of an accredited Physical Therapy program.
• Strong documentation and organizational skills.
• Excellent communication and interpersonal abilities.
• Willingness to travel to patient homes for evaluations and treatment.
• Flexibility regarding work locations throughout our service area.
