Job summary

Nexus at Westmont is seeking a Physical Therapist to develop individualized treatment plans to help residents regain independence, prevent decline, and safely participate in daily activities while enhancing quality of life.

You will assess evaluations and test results, develop treatment plans using various techniques, create rehabilitation programs, and administer therapies to relieve pain and improve function.

Qualifications

• Active/Valid Illinois license in Physical Therapy.
• SNF/Long term care experience preferred.
• Current knowledge of treatment practices.
• Knowledge of PCC Electronic Documentation System preferred.

Responsibilities

• Assess patient evaluations and test results.
• Develop treatment plans using a variety of treatment techniques.
• Create personalized rehabilitation programs for patients.
• Administering PT treatments to relieve pain and improve function.
