Personal Touch Home Health Care Services Inc. is seeking a part-time Physical Therapist to evaluate, develop, and implement individualized treatment plans to restore function, improve mobility, reduce pain, and promote independence.

Responsibilities include assessing patients, administering evidence-based therapies, monitoring progress, educating patients and caregivers, documenting care, coordinating with physicians, and contributing to discharge planning.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current or eligible for Illinois Physical Therapist license.
• CPR/BLS certification or ability to obtain.
• 1–2 years of clinical experience preferred; new graduates encouraged to apply.
• Strong assessment, communication, and critical thinking skills.
• Ability to work effectively within a collaborative healthcare environment.
