Job summary

Nexus at Elmwood in Elmwood Park, IL seeks a compassionate Physical Therapist to develop individualized treatment plans helping residents regain independence and participate safely in daily activities.

You will assess evaluations, create rehab programs, and administer therapy to relieve pain and restore function while promoting healing in a collaborative, team-oriented environment.

Qualifications

• Active/Valid Illinois license in Physical Therapy
• SNF/Long term care experience preferred
• Current knowledge of treatment practices
• Knowledge of PCC Electronic Documentation System preferred
