Job summary

AdvisaCare is seeking a licensed Physical Therapist in Michigan to provide patient-centered home health services within the community. You will develop personalized treatment plans, guide patients through therapeutic exercises, and monitor progress to maximize independence and mobility.

Qualifications

• Must hold a valid Physical Therapist license in Michigan.
• Minimum of one year home health care experience preferred.
• Strong communication skills with patients and families.
• Compassionate, patient-centered caregiver.
• Ability to work autonomously and manage own schedule.
• Reliable vehicle and valid driver's license for patient visits.

Responsibilities

• Develop and implement individualized treatment plans to improve mobility and function.
• Administer therapeutic exercises and interventions to restore independence and reduce pain.
• Assess and document patient progress, adjusting plans as needed.
• Collaborate with interdisciplinary care teams to ensure continuity of care.
• Educate patients, families, and caregivers on exercises and safety.
• Maintain accurate documentation and compliant with standards.

• Collaborate with interdisciplinary healthcare teams to ensure continuity of care and optimal outcomes
• Educate patients, families, and caregivers on exercises, safety techniques, and injury prevention
• Maintain accurate documentation and adhere to all legal, ethical, and professional standards

Requirements

What We’re Looking For:

• Licensed Physical Therapist - you must hold a valid license in the state of Michigan.
• Experience - while a minimum of one year in home health care is preferred, we’re open to fresh perspectives and new talent!
• Team Player - you should have excellent communication skills and the ability to engage with patients and families effectively.
