AdvisaCare is seeking a licensed Physical Therapist to join our Michigan team in Traverse City. You will develop individualized plans, work one-on-one with clients, and collaborate with a coordinated care team to restore mobility and independence.

Qualified candidates have a Michigan PT license, ideally one year of home health experience, strong communication skills, and a valid driver’s license with reliable transportation for client visits.

Qualifications

• Must hold a valid Michigan Physical Therapist license.
• Minimum one year of home health care experience preferred.
• Strong communication and teamwork skills.
• Ability to work autonomously and manage travel to patient locations.
• Valid driver’s license and reliable vehicle.

Responsibilities

• Develop and implement individualized treatment plans to improve mobility, strength, balance, and overall function.
• Perform therapeutic exercises and interventions to help clients regain independence and reduce pain.
• Assess and document patient progress, modifying treatment plans as needed.
• Collaborate with interdisciplinary healthcare teams to ensure continuity of care and optimal outcomes.
• Educate patients, families, and caregivers on exercises, safety techniques, and injury prevention.
• Maintain accurate documentation and adhere to all legal, ethical, and professional standards.

Requirements
