Job summary

AdvisaCare is hiring a Physical Therapist in the United States who will develop personalized treatment plans to improve mobility and function. You will work one-on-one with clients, educate families, and collaborate with an interdisciplinary team to ensure high-quality care in home settings.

The role emphasizes compassionate, patient-centered care, autonomous scheduling, and ongoing assessment to track progress and outcomes across diverse patient populations.

Qualifications

• Licensed PT must hold a valid license in Michigan.
• Minimum of one year in home health care preferred.
• Strong communication and engagement with patients and families.

Responsibilities

• Develop and implement individualized treatment plans focused on improving mobility, strength, balance, and overall function
• Perform therapeutic exercises and interventions to help clients regain independence and reduce pain
• Assess and document patient progress, modifying treatment plans as needed
• Collaborate with interdisciplinary healthcare teams to ensure continuity of care and optimal outcomes
• Educate patients, families, and caregivers on exercises, safety techniques, and injury prevention
• Maintain accurate documentation and adhere to all legal, ethical, and professional standards
