AdvisaCare in Michigan is seeking a licensed Physical Therapist to join our home health team. You will evaluate patients, create personalized treatment plans, and work one-on-one in patients' homes to restore mobility and independence.

This role requires Michigan licensure, strong communication, and reliable transportation for patient visits. We offer a supportive interdisciplinary team, competitive pay, and benefits.

Qualifications

• Licensed Physical Therapist in Michigan (active license).
• Experience in home health care preferred (1 year).
• Strong communication and interpersonal skills.
• Ability to work autonomously and manage schedules.
• Valid driver license and reliable vehicle for patient visits.
• Commitment to patient-centered, ethical care.

Responsibilities

• Develop and implement individualized treatment plans focused on improving mobility, strength, balance, and overall function.
• Perform therapeutic exercises and interventions to help clients regain independence and reduce pain.
• Assess and document patient progress, modifying treatment plans as needed.
• Collaborate with interdisciplinary healthcare teams to ensure continuity of care and optimal outcomes.
• Educate patients, families, and caregivers on exercises, safety techniques, and injury prevention.
• Maintain accurate documentation and adhere to all legal, ethical, and professional standards.
