Job summary

AdvisaCare is seeking a compassionate Full-Time Physical Therapist (PT) to deliver skilled home health therapy across Southeast Michigan, including Oakland, Macomb, Wayne and Washtenaw Counties. You will perform evaluations, develop plans of care, and coordinate with an interdisciplinary team in patients’ homes.

The ideal candidate holds an accredited PT degree, a Michigan license, and strong clinical skills with a preference for home health experience.

Qualifications

  • Degree from an accredited Physical Therapy program.
  • Current Michigan Physical Therapist license in good standing.
  • Strong clinical assessment, critical-thinking, and problem-solving skills.
  • Excellent patient and caregiver education skills.
  • Ability to work independently in the field while collaborating with the interdisciplinary team.
  • Reliable transportation, a valid driver’s license, and current automobile insurance.
  • Home health experience preferred.
  • Familiarity with Medicare/CMS home health documentation requirements preferred.

Responsibilities

  • Complete comprehensive physical therapy evaluations and identify functional limitations and rehabilitation needs.
  • Develop individualized plans of care, measurable goals, and appropriate treatment strategies.
  • Provide skilled therapeutic exercise, strengthening, gait training, balance training, transfer training, and functional mobility interventions.
  • Assess fall risk, home safety, and appropriate use of assistive devices.
  • Educate patients and caregivers on safety, fall prevention, mobility techniques, and Home Exercise Programs (HEPs).
  • Monitor patient response and progress throughout the episode of care.
  • Complete reassessments and modify treatment plans when clinically appropriate.
  • Coordinate care with physicians, caregivers, nursing, therapy disciplines, and other members of the interdisciplinary team.
  • Maintain accurate, timely, and compliant clinical documentation.
  • Provide services consistent with physician orders, the established plan of care, and applicable Medicare/CMS requirements.

We are seeking a compassionate, motivated Full-Time Physical Therapist (PT) to provide skilled home health therapy services throughout the Southeast Michigan area, including communities in Oakland, Macomb, Wayne and Washtenaw Counties.

What You’ll Do

As a Home Health Physical Therapist, you’ll work directly with patients in their homes to improve strength, mobility, balance, safety, and overall functional independence.

You will:

  • Complete comprehensive physical therapy evaluations and identify functional limitations and rehabilitation needs.
