Job summary

AdvisaCare is seeking a licensed Physical Therapist to deliver patient-centered care in the community. You will develop personalized treatment plans, work one-on-one in clients' homes, and educate patients and families on mobility, safety, and injury prevention.

In this role, you’ll complete evaluations, set goals of care, and oversee therapy services while collaborating with an interdisciplinary team. A compassionate, self-directed professional with a valid Michigan license is ideal.

Qualifications

• Active Michigan PT license in good standing.
• Home health care experience preferred (1 year).
• Strong communication and empathy with patients and families.

Responsibilities

• Develop and implement individualized treatment plans focused on improving mobility, strength, balance, and overall function.
• Perform therapeutic exercises and interventions to help clients regain independence and reduce pain.
• Assess and document patient progress, modifying treatment plans as needed.
• Collaborate with interdisciplinary healthcare teams to ensure continuity of care and optimal outcomes.
• Educate patients, families, and caregivers on exercises, safety techniques, and injury prevention.
• Maintain accurate documentation and adhere to all legal, ethical, and professional standards.

Requirements

What We?re Looking For:

• Licensed Physical Therapist - you must hold a valid license in the state of Michigan.
• Experience - while a minimum of one year in home health care is preferred, we?re open to fresh perspectives and new talent!
