AdvisaCare is seeking a licensed Physical Therapist to deliver one-on-one care in home health settings within Michigan. The role emphasizes evaluating patients, developing personalized treatment plans, and guiding families on mobility and safety to improve patient independence.

You will collaborate with an interdisciplinary team, document progress, and maintain high standards of patient-centered care while adhering to regulatory requirements.

Qualifications

• Licensed PT with current Michigan license.

Responsibilities

• Develop and implement individualized treatment plans to improve mobility, strength, balance, and function.
• Perform therapeutic exercises and interventions to help clients regain independence and reduce pain.
• Assess and document patient progress, adjusting plans as needed.
• Collaborate with interdisciplinary care teams to ensure continuity of care and outcomes.
• Educate patients, families, and caregivers on exercises, safety techniques, and injury prevention.
• Maintain accurate documentation and adhere to legal, ethical, and professional standards.

Requirements

• Licensed Physical Therapist - you must hold a valid license in the state of Michigan
• Experience - while a minimum of one year in home health care is preferred, we're open to fresh perspectives and new talent!
• Team Player - you should have excellent communication skills and the ability to engage with patients and families effectively
• Compassionate Caregiver - a genuine desire to help others and promote their independence is vital
• Self-Starter - the ability to work autonomously, manage your own schedule, and track patient progress is key
• Transportation - a valid driver's license and reliable vehicle for travel to patient locations
