Job summary

Vetted Solutions is seeking a licensed Physical Therapist to deliver personalized, in-home and senior living care to older adults, primarily through our Geriatric House Calls™ model. In this role you will provide one-on-one therapy in private residences and senior communities, develop treatment plans, document care in EMR, and collaborate with a multidisciplinary team to promote safety and aging in place.

Telehealth options are included to increase accessibility for patients.

Qualifications

• Valid Physical Therapy license and degree from an accredited program.
• Proficiency in basic computer and EMR systems.
• Current CPR certification.

• Providing one-on-one physical therapy sessions in private residences and senior communities.
• Conducting evaluations, developing individualized treatment plans, and delivering evidence-based therapy to enhance mobility and independence.
• Documenting care accurately within electronic medical records.
