Plan A Health seeks licensed Physical Therapists to deliver one-on-one geriatric care in private homes and senior living communities with a flexible schedule and the Geriatric House Calls model.

You will perform evaluations, create care plans, and deliver therapy while documenting progress in EMR. Collaboration with PTs, OTs, SLPs, and fitness specialists is expected, with education for patients and caregivers, plus telehealth options.

Qualifications

• Valid Physical Therapy license and degree from an accredited program.
• Proficiency with computer systems and EMR documentation.
• Current CPR certification.
• Ability to meet physical demands such as lifting, repositioning patients, and driving.
