Job summary

Apex Home Health, Hospice & Home Care seeks a licensed Physical Therapist to deliver one-on-one, in-home care for older adults in their homes and senior living communities.

You will perform evidence-based evaluations, create individualized plans, and provide treatment to improve mobility, independence, and quality of life, with telehealth options to increase flexibility and access. Collaborative, interdisciplinary teamwork is encouraged in a supportive environment.

Qualifications

• Valid Physical Therapy license and degree from an accredited program.
• Proficiency in basic computer skills, EMR systems, and current CPR certification.
• Ability to meet physical demands such as lifting, assisting with transfers, and performing therapeutic activities.

Responsibilities
